NHS Triage Form - England Only

The NHS (England Only) have introduced a triage check to ensure that a) triages are being carried out before a patient has treatment in the practice and b) so you can inform them of any remote triage/consultations you have performed. If you triage a patient and do not see them face-to-face for treatment, you can claim a Band 6.

We have added two new treatments that should appear in the same section as your exam treatment item.

If you are treating the patient in-practice

  1. add "Triage and Treatment" to your plan.
  2. once added, double-click on the triage item in the plan to show the item editor and go to the Item Form tab
  3. in the editor, choose the Patient Group which is the COVID-19 status of the patient and choose the primary reason for the call
  4. click "Finish Editing"
You can add this along with any other banded treatment you are performing on the patient and you will be awarded the appropriate band charge and UDAs as normal.

Remote triage of patient

If you are not seeing the patient face-to-face, you will need to complete a full triage form...
  1. start a new treatment plan
  2. add "Triage only" to the plan, then double-click on the treatment item
  3. Go to the Item Form tab
  4. Complete the relevant options on the form.
    1. The time of call, Patient Group and Reason for call are mandatory items to be completed
  5. Once completed, click "Finish Editing"
  6. You can now submit the claim by clicking the FP17 button. You will notice fewer options on the submit screen as fewer details are required on a Band 6 claim.
You must complete the time of call and patient group as a minimum
