Unable to Edit Forms (Locked)

Solution :

If you are unable to edit some or all forms in your OperaDDS account, the following reasons may apply:

  1. The form is a writeback-capable form, and is un-editable by the office to protect the integrity of the writeback functionality.
    These forms will appear with a green icon beside them as seen below.




    There are 2 forms that OperaDDS provides that can't be edited, these forms are the Patient Information and Medical History forms.

    The reason for that is...
    • One of the great features of the OperaDDS Forms solution is that these forms map to populate the information directly to the patient chart. Mapping to those fields requires that we not make any changes to those areas.
    • Since these forms are fixed and have A TON of information on them we've been able to optimize the output of the PDF so all the information fits on one page, as seen here ( Patient Information  | Medical History )
    • There is an exception to this rule. Eaglesoft has a customizable Medical History Form within the Eaglesoft application that OperaDDS can recreate as an online form. 
    Don't despair, there is an option.  You can create your own Patient Information or Medical History form that can be customized to be EXACTLY what you want. HOWEVER...
    • These forms won't be mapped back to auto populate the patient information and medical history areas of the patient chart in your practice management system.
    • The PDF output of a customized Patient Information or Medical history form can get very lengthy since they both have a ton of information. 
    One other option that you have is that you can simply create an additional page for additional questions to either the Patient Information or Medical History to capture any additional questions you might have and you can add that to the Patient Intake Packet.
