OperaDDS - Using Forms on iPad and Android Tablets

Please use the following guides to set up and use OperaDDS forms on your office's iPad and/or Android tablets.


iPad and Android Tablet Sizes



Turning off AutoFill - HIGHLY Recommended for Patient Information Security


iPad Instructions
  1. From the Home Screen, click the Settings App
  2. In the Settings App, click Safari | Click Passwords & Autofill
  3. Turn OFF all options
  4. Repeat on all iPads that will use Forms


Android Instructions
  1. From the Home Screen, click the Google Chrome App
  2. Click the three dots in the upper right corner | Click Settings
  3. Turn OFF Auto-fill forms option
  4. Repeat on all Android tablets that will use Forms


Adding Forms Launcher to the Home Screen


      iPad Instructions
  1. From the Home Screen, click the Safari App
  2. Go to www.operadds.com and log in with your username and password
  3. Click on Forms tab | Click Blank Forms
  4. Click Forms Launcher
  5. In the new window, click the Share icon [^] | Click Add To Home Screen
  6. Repeat on all iPads that will use Forms


      Android Instructions
  1. From the Home Screen, click the Google Chrome App
  2. Go to www.operadds.com and log in with your username and password
  3. Click on Forms tab | Click Blank Forms
  4. Click Forms Launcher
  5. In the new window, click the Settings icon [...] | Click Add To Home Screen
  6. Repeat on all Android tablets that will use Forms


Adding Forms Updater to the Home Screen


      iPad Instructions
  1. From the Home Screen, click the Safari App
  2. Go to www.operadds.com and log in with your username and password
  3. Click on Forms tab | Click Blank Forms
  4. Click Forms Updater
  5. In the new window, click the Share icon [^] | Click Add To Home Screen
  6. Repeat on all iPads that will use Forms


      Android Instructions
  1. From the Home Screen, click the Google Chrome App
  2. Go to www.operadds.com and log in with your username and password
  3. Click on Forms tab | Click Blank Forms
  4. Click Forms Updater
  5. In the new window, click the Settings icon [...] | Click Add To Home Screen
  6. Repeat on all Android tablets that will use Forms


Using Forms on iPad and Android Tablets


      Filling Out New Forms
  1. Click on Forms Launcher on the Home Screen of your iPad/Android Tablet
  2. Click the form(s) or form packet desired
  3. Click Launch


      Updating Existing OperaDDS Forms
  1. Click on Forms Updater on the Home Screen of your iPad/Android Tablet
  2. Log In with your username/password
    • This is a required step every time you open Forms Updater, due to accessing Protected Health Information.
  3. Type in the patient's name | Click Add To Form
    • Here you will see a list of the patient's previously filled forms at the top and a list of your office's BLANK forms at the bottom
  4. Click the checkbox next to the forms you want the patient to update | Click Launch
    • If a form is chosen from the top of the list (previously filled forms), the information will pre-fill with the old information entered by the patient in the past.
    • Forms that appear in the updater are forms that have been LINKED to a patient on your Completed Forms page in the past.
