How to: Send a Web Survey through Chat

Web Surveys are used to request feedback from existing clients/contacts. They are completely customize-able with answer options such as check boxes, multiple choice, and open ended questions. We also offer the ability for clients to Upload up to 10 mb's worth .pdf, .jpg, and .png files to use as answers to survey questions.
Note:  Web Surveys do not offer the ability to use a clients preferred language. A separate survey can be created in another language and then send to clients who use that preferred language.

Note:  Web Surveys must be enabled on an account prior to use. Please reach out to Teletask staff to enable this feature.
Note: There are 3 "Types" of surveys, 1) An anonymous Survey. If a client responds to this type, it will not be associated with their phone number 2) A Tracked Type. If a client responds to this type, it will be associated with their phone number. 3) Tracked Type where responses appear in Chat. If a client responds to this type, it will be associated with the client profile, and the results, including any files upload will appear on chat.

  1. Create a Web Survey.

  2. Click on Chats   in the top menu bar.

  3. Browse or Search for the client that will receive the survey. You can search by name or phone number.



  4. To add the survey, click on the Web Survey icon . A list of your available surveys will appear, select the survey that you want to send.



  5. The link for the survey will appear in the chat. You can then choose Send or Send + Close to send the link to your client.



Survey Results

If a survey type is "Tracked Type where responses appear in Chat", the results will immediately be pushed to chat. Otherwise, to view the results of a survey follow the steps below.

  1. Log-in to: https://hms.teletask.com/, your email address is your username.

  2. Click on Reports   in the top menu bar.

  3. Select Web Survey  from Reports section.

  4. Use the drop-down menu next to Web Survey to pick your survey from the list.



  5. Select a Date Range for responses.



  6. Press the Generate Report button  to view the results.
    Note: since this survey is anonymous the First and Last Name are left blank.


    Clicking Link will open up the image file that was attached.
Once the results are generated, you can export them as a .csv Excel file  or choose to print them .

When you send the survey to a client, they will click on the link and the survey will open up. The client can then make their selections and submit the survey.

When you click on the link, it will open the survey.



When you select the choose file option, you can either take a photo or add an existing photo.



After they click submit they will receive this message.
