How to add Patient Questionnaire

When uploading a study you will be prompted to add the patient's questionnaire information. This should appear as a pop-up window during the physical uploading of data from your ARES device. After uploading a study it will remain in a "ProcessPending" > "Processing" status for up to 30 minutes, and during this time you will not be able to add a questionnaire. Here are some questions you might have regarding this process- 

How do I add the questionnaire to a study after it has been uploaded?
The study will have to be in a qualifying status without having been interpreted(signed), and located in one of two places- "Pending Studies Queue" or "Reports" > "Study Reports"
  • Go to  Home Sleep Testing Pending Studies Queue

  • Click Search to generate a list of studies in a pending status
  • Locate your patient's name and click the Green Plus Sign  on the patient name
  • Click Add Questionnaire, enter the information and click  Save 

  • You can also access "Available" studies in "Reports" > "Study Reports" and follow the same instructions above.

When would I not be able to add a questionnaire to a study?
The questionnaire can only be added if the study has NOT been interpreted by your assigned reading physician AND it is in a "Client Tech Edit/Tech Edit/Interpretation Pending" or "Available" status.  

What happens if I cancel the questionnaire pop-up? 
Unfortunately, you will have to wait for the study to appear in the Pending Studies Queue. Once available you can follow the instructions for adding a questionnaire .

The pop-up window did not appear. What can I do?
If the pop-up window did not appear, first confirm the pop-up did not open behind another window. Then confirm your Pop-Up Blocker Settings are configured correctly. Unfortunately, you will have to wait for the study to appear in the Pending Studies Queue. Once available you can follow the instructions for adding a questionnaire .
