Digital Outcome Assessment Instructions
Log in to your WebExercises Account and the left hand menu, you will see a new page labeled “Questionnaires". Click on this to see a list of patient questionnaires that is ready to be sent to patient. Once it is complete it will display scored and ready to print.

What questionnaires are available?
- The Bournemouth Back questionnaire (BQ) is a comprehensive multi-dimensional core outcome tool assessing patients’ outcome of care in a routine clinical setting.
- The Bournemouth Neck questionnaire (BQ) is a comprehensive multi-dimensional core outcome tool assessing patients’ outcome of care in a routine clinical setting.
- The Physical Activity Readiness Questionnaire (PAR-Q) is a screening tool and consists of 7 closed-ended questions that assess an individual’s level of physical fitness and ability to engage in physical activity.
- The SF-36 Health Survey is a 36-item, patient-reported survey of patient health. The SF-36 measures eight scales: Physical functioning (PF), Role limitations due to physical health (RP), Role limitations due to emotional problems (RE), Energy/fatigue (VT), Emotional well being (EWB), Social functioning (SF), Bodily pain (BP), and General health (GH).
Are the questionnaires included in my WebExercises Rx membership?
Yes, the 4 questionnaires are included in your monthly or annual membership.
How do I send a questionnaire to a patient and how to they access it?
To send 1 to 4 questionnaires, navigate to the patient account the same way you would to send an exercise program. You will see a new section "Patient Questionnaires" with a drop down menu to select the questionnaire to send. The "Required" check box requires the patient to complete the questionnaire prior to viewing their exercise Rx whether online or through the patient mobile app.
Will I be notified when a patient completes a questionnaire?
Yes you will see a green notification on Questionnaires in the left hand menu.
